Gynecomastia vs. Pseudogynecomastia: How to Tell the Difference

"Man boobs" is one label covering two different things — and knowing which one you have changes what will actually help. Gynecomastia is enlargement of glandular breast tissue, driven by hormones. Pseudogynecomastia is fat accumulation on the chest, driven by overall body fat. They can look similar in a t-shirt, but they behave very differently.

The quick self-check

Lie back and gently pinch the tissue directly behind the nipple between your thumb and finger.

A firm, rubbery, disc-like lump behind the nipple — often a little tender — suggests glandular tissue: gynecomastia.
Soft tissue that feels the same as the fat elsewhere on your chest or stomach, with no distinct firm disc, suggests pseudogynecomastia.

Other clues: gynecomastia is often centered tightly under the areola and can affect one side more than the other; pseudogynecomastia is usually symmetrical and spreads across the whole chest, tracking your overall weight. This self-check is a starting point, not a diagnosis — a doctor can confirm by examination.

What causes each

Gynecomastia comes from a shifted estrogen–testosterone balance. It's extremely common during puberty (usually resolving on its own within a couple of years) and again in middle and older age. Certain medications, anabolic steroids, heavy alcohol use, cannabis, and some medical conditions can also trigger it.

Pseudogynecomastia is simpler: it's where your body happens to store fat. Genetics decide the location; overall body-fat percentage decides the amount.

What actually helps

If it's pseudogynecomastia: fat loss works. You can't spot-reduce chest fat specifically, but as overall body fat drops through a calorie deficit and strength training, chest fat drops with it. Building the pecs (presses, dips, flyes) improves chest shape as the fat reduces.

If it's gynecomastia: exercise won't shrink glandular tissue — this is the hard truth that saves men years of frustration in the gym. Options are: waiting (especially for teens, where it usually resolves), treating an underlying cause (medication change, cutting alcohol), surgery for lasting removal, or managing appearance day to day.

See a doctor promptly if the tissue is new and growing quickly, painful, one-sided, hard or fixed in place, or accompanied by nipple discharge — and any time you simply want certainty. Male breast changes are usually benign, but they're always worth confirming.
